This website is dedicated to the observance and compliance of the Code of Conduct and Ethics (the “Code”). If you have any genuine concerns regarding compliance with this Code, or have reasonable grounds to believe that a breach of this Code has been committed or might be committed, please raise your good faith concern directly by submitting details as it relates to your complaint/concern in the form below.

When completing the form, please provide as much information relating to the incident as possible, such as who, what, and where at a minimum. Sufficient information is necessary in order to allow the Ethics Committee to conduct a proper and thorough investigation of your complaint/concern.

This form can be completed anonymously, but you are free to disclose your name should you choose to do so, and your privacy will always be respected. If you disclose your personal information, please be aware that the Ethics Committee might choose to get in touch with you to discuss your complaint/concern further. Any and all information disclosed hereunder shall be treated fairly and with integrity, and only the people who need to have knowledge of or access to this information shall have access to it.

No person shall be treated unfairly or victimized as a result of making any disclosure under this Code, provided that no disclosures are malicious or vexatious.
